Elena did not go to the police.

Not first.

She went to her hospital.

Powerful women survived by knowing which rooms mattered.

The boardroom mattered.

The security office mattered more.

The basement server room mattered most.

She entered St. Agnes through the staff garage at 6:14 a.m., wearing scrubs, a wool coat, and the expression that made residents confess mistakes before she asked.

Rafe walked three paces behind.

She had allowed one guard.

Only one.

Dominic had not liked it.

Dominic had still obeyed.

That mattered.

Elena hated that it mattered.

The head of hospital security, Graham Pike, met her near the elevators.

His eyes flicked to Rafe.

“No outside personnel.”

Elena held up her badge.

“My patient was shot after a breach.”

“We are investigating.”

“I am investigating.”

“That is not hospital protocol.”

“Neither is assassination.”

Pike’s mouth thinned.

“Dr. Vale.”

She stepped closer.

“You gave someone access.”

His face changed too fast.

There.

A tell.

Rafe shifted.

Elena lifted one hand.

He stopped.

Pike recovered.

“That accusation is dangerous.”

“So was the bullet.”

“I suggest you speak to legal.”

“I suggest you start sweating quietly.”

His eyes hardened.

The elevator opened behind him.

Luca Santoro stepped out.

Elena had seen him once at a gala.

Gray hair.

Polished cane.

Smile like a sealed coffin.

“Dr. Vale.”

Pike went pale.

Rafe’s hand moved under his jacket.

Elena did not blink.

“Mr. Santoro.”

“I came to check on the hospital.”

“How civic.”

His smile widened.

“Tragedy invites concern.”

“Murder attempts invite prison.”

Pike whispered,

“Doctor.”

Santoro laughed softly.

“I admire precision.”

“I admire convictions.”

“Then we both enjoy rare things.”

He stepped closer.

Rafe moved.

Elena did not.

Santoro’s eyes dropped to her scar.

“Dominic always did choose breakable women.”

Elena smiled.

“You must not know many surgeons.”

His cane tapped once.

“You saved the boy.”

“Yes.”

“You saved Dominic too.”

“Yes.”

“Sentimental.”

“Professional.”

Santoro leaned in.

His cologne smelled like cedar and rot.

“You should have let one die.”

The hallway went silent.

Elena’s pulse slowed.

Surgeons knew that feeling.

The body preparing for the cut.

“Which one scares you more?”

Santoro’s smile faded.

“Excuse me?”

“Nico alive, or Dominic grateful?”

His gaze sharpened.

“There she is.”

Elena’s stomach tightened.

He knew too much.

Santoro looked toward Rafe.

“Tell your employer I want what Matteo stole.”

Elena kept her face blank.

“Then ask Matteo.”

“He is unavailable.”

“Many dead men are.”

Santoro’s eyes cooled.

“The chip, Doctor.”

Pike looked down.

Guilty.

Terrified.

Elena saw it.

Santoro saw her see it.

The air changed.

Rafe drew his gun.

Too late.

Pike grabbed Elena from behind and pressed a blade to her throat.

Santoro sighed.

“Messy.”

Elena did not struggle.

A shaking man with a knife was a math problem.

She could solve math.

Rafe froze.

Pike’s breath burned against her ear.

“I’m sorry.”

“No, you aren’t.”

“They have my daughter.”

That hit.

Elena went still.

Santoro looked bored.

“Everyone has daughters.”

Elena’s fear sharpened into something colder.

“Her name?”

Pike trembled.

“What?”

“Your daughter.”

“Amelia.”

“How old?”

“Seven.”

“Does she have asthma?”

His grip faltered.

“How did you—”

“You carry a rescue inhaler on the wrong side. Not yours.”

Santoro’s smile vanished.

Elena continued softly.

“Let me help her.”

Pike made a broken sound.

Santoro raised his cane.

Rafe fired.

The bullet struck Santoro’s cane, shattering the hidden barrel inside.

Pike dropped the knife.

Elena turned and drove her elbow into his woundless side.

He folded.

Rafe dragged her back.

Santoro’s guards filled the hallway.

Then police sirens screamed outside.

Elena looked at Santoro.

His face finally changed.

She touched the tiny recorder clipped beneath her coat collar.

“Convictions are rare.”

Her voice shook only after the sentence.

“But recordings help.”

Santoro’s eyes turned murderous.

Police stormed the corridor.

Pike sobbed on the floor.

Rafe spoke into his phone.

“Package exposed.”

Elena leaned against the wall.

Her throat burned where the knife had kissed it.

Her phone buzzed.

A text from Dominic.

Nico is asking for you.

Then another.

So am I.

Elena closed her eyes.

The wound at her throat stung.

The wound in her hand burned.

For the first time, both pointed home.
